Form tab icons come from your [[Images#Project_images|project images]], from the Tabs directory. The app uses an image with the same name as the '''Name''' of the tab in the '''Properties''' pane in Woodford. If such an image does not exist, it looks for an image called "General", again in the Tabs directory. If the image does not exist, no icon is shown.
You can then use the image picker on the '''Properties''' pane to select the icon for your tab. Alternatively, you can even upload your own custom image: click '''Remove image''' (this restores the default icon for the tab; the old icon is not deleted) and then click '''Add image''' to upload a new one.
